The staff is very nice, all the amenities are there. A walking distance from the shibuya scramble.
The entrance was a bit hidden, I passed by it twice without noticing it, the first time. You need to look up to see the vertical sign.

Nothing. It's very expensive for this location and the age of the hotel. Only the bathroom is halfway decent. Everything else is a nightmare.
Everything. The staff is rude, they try to sell you everything, even a bottle of water that costs 20 cents across the street. The staff specifically hates Americans, they are rude and unfriendly. Not just impolite, but truly rude. Every time you leave, you have to hand in your key, but when you return, they pretend not to know you and start the check-in process and ask who you are – every single time you leave your room! It's pure harassment.

Very nice staff. Tidy clean hotel. I had a single room on the top floor. It was very small as you would expect in Tokyo, in the centre. The desk and chair at one end of the room were so close to the bed you could hardly sit in the chair. Rooms cleaned every day if you want. Various extras are supplied like toothbrush and pyjamas (although I didn't use them) and various other things. No noise, so nice and quiet. This is one of those hotels where you hand your key in to reception when you go out, and it's an old fashioned key rather than a swipe card. But you get a free bottle of water every day and there was coffee at reception in the morning. There's no cafe or food offered but there is a decent cafe next door, with a number of salarymen working. Obviously the location makes it easy to get places. The shower is connected to the taps, so you have to switch it over and then adjust the taps to get the right temperature each time you use it. The one I had was set in a mini bath with a hose and shower head. No complaints as the price I got was very reasonable.
渋谷駅から少し歩きますが、青山に歩いて行ける立地がとても魅力的です。近くにカフェがたくさんあって、朝食がなくても不便さはありません。
